We C.U. so it’s easy to complete
a C.U. Membership Application. Here are some important things to remember
while you’re filling in the form, to ensure a successful application:
- Please ensure the application is completed
in its entirety and that you’ve signed it.
- Remember to enter your social insurance
number. It’s mandatory.
- You will be expected to purchase10 shares
at $10.00 each. This is also mandatory. Please return this application
with a cheque in the amount of $100.00 payable to the Ontario Civil
Service Credit Union Ltd. to purchase your ten $10.00 shares.
- Legible photocopies (front & back)
of two (2) pieces of original, current & non-expired identification
are required. One piece is to bear your photo and signature. The following
are acceptable forms of identification: Driver’s License, Ministry
Card, Credit Card, Birth Certificate or Passport and proof of employment.
Please make sure that the photocopies are fully legible and clear.
You may need to set the copier button to a ‘lighter’ setting
to ensure that it is legible.
- Please ensure you answer the Security Code
& Security Code Question & Answer sections.
- Answers for the anti-money laundering questions
(Q1 & Q2) are required. If response is yes to either of these
questions please contact C.U. Member Assistance for help in completing
your application.
- When signing, please ensure that your signature
is contained within the box on the form and that no part of your signature
touches the lines. (We also request that you do not date the
signature card and that it be completed with a BLACK INKED PEN.)
