Identification (ID) requested: Legible, clear photocopies (front & back) of two (2) pieces of original, current and non-expired ID are required. You may need to set the copier button to a ‘lighter’ setting to ensure that it is legible. One piece of ID must bear your photo and signature. Of the 2 pieces of ID, one must be a Primary source of ID and the other Secondary, as follows:
Primary Identification:
- Ontario Drivers License
- Canadian Passport
- Certificate of Canadian Citizenship with photo
- Canadian Certification of Naturalization in the form of a paper document or card but not a commemorative issue
- Permanent Resident Card or Citizenship and Immigration Canada Form IMM 1000, IMM 1442 or IMM 5292
- Certificate of Indian Status issued by the Government of Canada
- BYID (formerly Age of Majority card) issued by the province of Ontario
- Firearms License issued by the Government of Canada
Secondary Identification:
- Credit card of known retailer with your name embossed and your signature on the back of the card
- Employee identity card, issued by Provincial, Federal or Municipal government, bearing your photograph
- Canadian Birth Certificate
- Social Insurance Number Card (SIN)
- Old Age Security card issued by the Government of Canada
- Canadian Driver’s license issued from another province
- Foreign Passport
- Canadian National Institute for the Blind (CNIB) client card bearing your photo and signature
- Employee identity card issued by your employer that is well known in the community bearing your photograph
- Introduction letter from a previous credit union, bank or other known financial institution your signature
- Canadian University or College Student Card with photo
